If you cannot speak for yourself, your phone can.

A lock-screen medical ID for iPhone and Android. Your allergies, conditions and emergency contact show on the lock screen, and a QR code opens your full emergency card. No unlocking needed.

Make my Medical ID

Built by Harvey Castro, MD, MBA, a board-certified emergency physician. In an emergency, call 911.

Who it helps

1
You cannot answer questionsUnconscious, seizing, confused after a fall, or a stroke has taken your words. The paramedic sees your allergies and conditions on the lock screen in seconds.
2
Your phone is foundWhoever finds it can scan the code and call your emergency contact to return it, without getting into your phone.
3
Someone you loveMake one for a parent with dementia, a child with a peanut allergy, or a teen athlete. Print the wallet card and the sticker for a bag, helmet or car seat.

Make your Medical ID

Fill in only what a stranger helping you would need. Everything stays in this browser. Your phone previews update as you type.

Anyone who scans the code can read what you enter. That is the point in an emergency. Do not include your Social Security number, insurance ID, address or anything you would not want a stranger to see.

Put it on your lock screen

1
Save the imageDownload the wallpaper for your phone. On a phone, it saves to Photos or your Downloads folder.
2
iPhoneOpen Settings, then Wallpaper, then Add New Wallpaper. Choose Photos, pick the image, tap Add, then Set as Wallpaper Pair or choose a different home screen.
3
AndroidOpen the image in Photos or Gallery, tap the menu, choose Use as or Set as wallpaper, then pick Lock screen. Steps vary a little by phone maker.
Also turn on your phone's built-in emergency info. On iPhone, open the Health app, set up Medical ID and turn on Show When Locked. On Android, open Settings and search for "emergency information." Responders are trained to look there, and it works with no internet.

Questions

Where is my information stored?

Inside the QR code itself. When someone scans it, their phone opens qrmedic.com/id and reads your information from the part of the link after the # sign. Browsers do not send that part to any server, so QR Medic never receives it. Your form is saved only in this browser on this device.

What if my medications change?

Come back to this page on the same device, edit the form, and download a new wallpaper. The old code keeps showing the old information, so replace printed cards and stickers too. Review it every six months and after any hospital stay.

Does the scan need internet?

Opening the full card does. The most important facts (allergies, conditions, emergency contact) are printed on the wallpaper itself, so they are readable even with no signal.

Will paramedics look at my lock screen?

Many do, but practice varies. That is why the key facts are printed large on the screen, and why you should also set up your phone's built-in Medical ID. Wearing a medical alert bracelet is still wise for serious allergies, diabetes, epilepsy and blood thinners.

Is this a medical device or HIPAA service?

No. It is a free personal information tool. It does not diagnose, monitor, alert anyone or store health records. Nothing you type reaches QR Medic.
